Greg Mohr, Cr Shield's nominated family spokesperson, updated his condition at the ordinary council meeting on Monday.
Cr Shields had been flown to the Royal Prince Alfred Hospital earlier this month for emergency surgery to his lung due to "life threatening" medical complications, and a decline in health, Cr Mohr said.
Cr Mohr said Cr Shields had returned to Dubbo Hospital on Saturday night, where he was still receiving treatment for infection.
"He's still currently receiving treatment at Dubbo hospital for infections," Cr Mohr said.
"He is having trouble breathing, obviously [because of] having some of his lung removed.
"He's nowhere near recovered fully. As I've said before, this will take a lot of time."
Cr Shields was found at his home in Dubbo on Wednesday, March 31, and placed on life support at Dubbo Hospital.
Earlier that day a public statement signed by six councillors was released, calling for Cr Shields to resign and for members of the community who had concerns about their interactions with any councillor to come forward.
Before the council announced later that afternoon that the mayor was taking indefinite medical leave, Cr Shields commented on his Facebook page that he "had only ever done what is right for Dubbo".
Deputy mayor Stephen Lawrence is acting in the mayoral role.
