Greg Mohr, Cr Shield's nominated family spokesperson, said he underwent surgery on Saturday morning after medical complications and a decline in health.
"[Cr Shields] is now in intensive care at RPA and will remain there for some time," Cr Mohr said.
"Ben's condition is critical but stable."
Cr Shields was found unconscious at his home in Dubbo on Wednesday, March 31, and placed on life support at Dubbo Hospital.
Earlier that day a public statement signed by six councillors was released, calling for Cr Shields to resign and for members of the community who had concerns about their interactions with any councillor to come forward.
Before the council announced later that afternoon that the mayor was taking indefinite medical leave, Cr Shields commented on his Facebook page that he "had only ever done what is right for Dubbo".
Deputy mayor Stephen Lawrence is acting in the mayoral role.
State Member for Dubbo Dugald Saunders addressed his "serious concerns" in a letter sent to the council's interim CEO Dean Frost last Monday, and suggested an independent investigator be appointed to conduct a review of the council's operations.
Dubbo Regional Council will hold an extraordinary meeting on Monday to vote and discuss the reputation of council and the protection of the community.
Councillors will meet at 12pm to debate and vote on eight recommendations put forward by acting mayor Stephen Lawrence, and supported by a majority of other councillors.
