The man received lacerations to his legs and he complained of chest pains.
The accident occurred at the Greghamstown Road level crossing, near Blayney, at 11.50am on Sunday.
The Dubbo bound XPT struck a glancing blow to the front of the vehicle causing moderate damage to the car.Neither the driver of the train nor any of the 122 passengers were injured.
Train services were disrupted for one hour as emergency services worked to clear the site.
Police have said investigations are continuing.
