The man died soon after arriving at Wellington Hospital by ambulance on Wednesday.
"This gentleman had received two doses of COVID-19 vaccine, he had significant underlying health conditions then contracted COVID-19 around nine days after his second vaccination and didn't have the full protection," Western NSW LHD Chief Executive Scott McLachlan said.
"Out of respect we're not going to be giving any more details about this case."

The region saw seven cases up to 8pm Wednesday with three in Bourke, two in Dubbo, one each in Wellington and Walgett, but the latter is being investigated as a potential mistake in reporting.
The Dubbo LGA's vaccination rate is higher than 73 per cent double dose and 93 per cent for single dose, but the health authority is concerned that the rate has slowed across the region.
"(Wednesday) saw the lowest number of people vaccinated over the last two months, only 375 people came to get a vaccination," Mr McLachlan said. "We know with high rates of vaccination in the region it was going to be something that we'll see in coming days and weeks, but we're incredibly concerned that we've still got 30 per cent of the community to have a second dose and at least 10 per cent to have a first dose."
There are 14 people in hospital including nine in Dubbo. Three people are in ICU and two are being ventilated.
