Emergency services were called to a site three kilometres north of Eumungerie about 1pm on Monday.
Orana Local Area Command Acting Inspector Andrew McKittrick said the sedan was travelling south along the Newell Highway before it left the road and hit a tree.
"As a result of the accident, one female is deceased," he said.
"There were three other occupants of the vehicle. Two were taken by road ambulance to Dubbo Hospital and one male has been airlifted to Westmead Hospital."
The female passenger who died at the scene was travelling in the rear of the car. A male, also travelling in the rear of the car, was airlifted to Westmead Hospital in a serious condition.
The two females travelling in the front of the car were taken to Dubbo Base Hospital in a stable condition.
Diversions were put in place for about five hours after the crash prompted the closure of the Newell Highway between Gilgandra and Dubbo, with the Traffic Management Centre warning motorists to allow extra time for travel.
The highway reopened about 5.30pm.
