About 5.15am on Monday, March 23, emergency services were called to a home on Merrygoen Road at Neilrex, about 60km south of Coonabarabran.
They were responding to a report of a toddler who had been coughing, and who appeared not to be breathing.
The child was taken to Coolah District Hospital, where he was pronounced dead.
Police from Mudgee Local Area Command formed Strike Force Toora to investigate the matter.
They set up a crime scene at the Merrygoen Road property on Wednesday, with assistance from the Forensic Services Group.
Police say inquiries to date suggest the death is suspicious, and police are now appealing for information from the public to progress the investigation.
Anyone with information they believe can help investigators is asked to contact Mudgee Detectives on (02) 6372 8599 or Crime Stoppers on 1800 333 000.
Strike Force Toora is an ongoing investigation by police from Mudgee Local Area Command.
They are being assisted by officers from State Crime Command's Homicide Squad.
