The collision on the Newell Highway claimed the lives of a 73-year-old woman and a 76-year-old man.
Police were yesterday continuing to investigate the crash that happened on Saturday afternoon and further details about the victims had not yet been released.
The elderly woman, who was a passenger in the car, died at the scene.
The car's 76-year-old male driver was taken by ambulance to Dubbo Hospital, suffering a number of injuries. Police confirmed he died early yesterday morning, the fourth fatality on NSW roads at the weekend.
The male driver of the truck was not injured and was taken to Dubbo Hospital for mandatory blood and urine testing, police reported.
Emergency services were called to the crash scene, near the highway's intersection with Blackbutt Road on the city's southern outskirts, about 3.30pm.
Dubbo Rescue Squad captain Tony Hare said they responded to a call of people trapped in a motor vehicle.
Seven volunteer rescuers attended the scene and were there for about four hours, he said. The highway was closed in both directions and traffic diversions were put in place for a number of hours, before the Transport Management Centre advised it had reopened.
Fire and Rescue NSW crews were on scene for a number of hours.
