Mr McLachlan, currently director of operations for the Hunter New England Local Health District, has held various leadership roles in health for the past 16 years and will take up the new appointment in February.
"Scott McLachlan's appointment to the role comes at an exciting time for the local health district with the redevelopment of Dubbo Base Hospital now underway," Mr Grant said.
"I'm looking forward to working closely with Mr McLachlan to ensure that health services in the region continue to deliver better outcomes for patients."
Mr McLachlan replaces the highly respected Ron Dunham who resigned from the role because of family circumstances in August.
The Western NSW Local Health District has been well served during the interim period by Dr Pim Allen.
