A b-double truck and bus collided on the Newell Highway, five kilometres south of Dubbo, at 7.40am on Tuesday.
The crash killed the 36-year-old bus driver and left a 47-year male passenger on the bus and the 69-year-old male truck driver injured.
NSW Ambulance Inspector Chris Wilson said paramedics were faced with a horrific scene.
"The bus driver sustained significant injuries to his lower limbs and as a result of those injuries he died at the scene despite the efforts of paramedics," he said.
The injured bus passenger and truck driver were assessed by paramedics at the scene and were then transported to Dubbo Base Hospital.
NSW Police Acting Superintendent Keith Ridley said the vehicles impacted quite heavily.
"It was a very chaotic scene, obviously a truck and a bus impacting at the speed limit created quite a bit of carnage," he said.
"The injury to one of the drivers was quite severe and it was quite a harrowing scene for all emergency first responders."
Supt Ridley said the truck driver was local, but the driver and passenger in the bus were "from out of town".
"At this stage we don't have any indication as to what occurred ... we've still got the investigators on the scene," he said.
"We are asking people to ensure they have plenty of breaks in relation to managing their fatigue on the road as well as managing their speed to the speed limit."
The Newell Highway remained closed to motorists until 4pm.
